At the Department of Family Medicine, we are committed to providing outstanding

the inpatient, hospice and home setting
Our clinicians strive to provide a patient centered medical home
for every patient we care for.
The patient centered medical home is a health care setting that
facilitates partnerships between individual patients, and their personal
physicians, and when appropriate, the patient's family.
Through a medical home, patients receive primary care that is accessible,
continuous, comprehensive, family-centered, coordinated, compassionate, and
culturally effective.
